Qualifying for Student Disability Accommodations and/or Services

1. You must be admitted to Â鶹´«Ã½ State in order to use the Office of Student Disability Services. If you have questions about admission to Â鶹´«Ã½ State, please contact the Office of Recruitment at 1.800.272.4Â鶹´«Ã½.

2. You must provide Documentation of your disability.  Documentation guidelines can be found for each type of accommodation under Accommodations and Services.

Registering with the Office of Student Disability Services

To register with the Office of Student Disability Services at Â鶹´«Ã½ State University, a student must:

1. Be admitted to Â鶹´«Ã½ State University and receive an M number (issued after admission).
   

2. Complete the .

3. Provide Documentation of your disability.  Documentation guidelines can be found for each type of accommodation under Accommodations and Services.

4. After you have completed the SDS Registration Form and provided Documentation, our office will contact you to schedule an intake appointment with the Director or Associate Director.
